\u6709\u4e9b\u4eba\u6703\u958b\u59cb\u7761\u4e0d\u597d\u3001\u5bb9\u6613\u7dca\u5f35\u3001\u958b\u8eca\u6642\u4e0d\u5b89\uff0c\u6216\u89ba\u5f97\u6574\u500b\u4eba\u4e00\u76f4\u8655\u5728\u4e00\u7a2e\u9084\u6c92\u653e\u9b06\u4e0b\u4f86\u7684\u72c0\u614b\u3002\n \u6848\u4ef6\u7de8\u865f\uff08claim number\uff09<\/strong><\/li>\n      <li>\u60a8\u7684 <strong>PHN\uff08\u5065\u4fdd\u5361\u865f \/ Care Card\uff09<\/strong><\/li>\n    <\/ul>\n\n    <p>\n      \u6e96\u5099\u597d\u9019\u5169\u9805\u8cc7\u6599\u5f8c\uff0c\u5176\u9918\u6d41\u7a0b\u90fd\u53ef\u4ee5\u7531\u6211\u5011\u5354\u52a9\u8655\u7406\uff0c\u4e26\u900f\u904e <strong>\u76f4\u63a5\u7406\u8ce0\uff08direct billing\uff09<\/strong> \u9032\u884c\u3002\n    <\/p>\n  <\/div>\n\n  <!-- REFERRAL -->\n  <h2
